Dust in the Cooler & Fans
This is the number one cause, period. I've seen it a hundred times. You're gaming, rendering, or running a stress test, and suddenly your CPU drops from 4.5 GHz to 800 MHz. Temps hit 95°C in seconds. Open your case, and there it is: a thick layer of dust on the heatsink fins and fan blades. That dust acts like insulation. Air can't pass through. Heat stays trapped.
Fix it: Get a can of compressed air (or an electric duster if you're fancy). Shut down your PC. Unplug it. Hold the fans still with a finger to stop them spinning (spinning them with air can damage bearings). Blow out the dust from the heatsink, fan, and case vents. Focus on the CPU cooler fins. If the dust is caked on, use a soft brush to loosen it first. Takes 5 minutes. Do it every 6 months.
Pro tip: If you have a liquid cooler (AIO), check the radiator fins too. Same problem. Dust there kills your cooling. Don't forget the PSU fan either.
Bad Thermal Paste or Poor Cooler Contact

Check in BIOS or software: SpeedFan, HWMonitor, or your motherboard's app. Look for fan RPM. If a fan shows 0 RPM, it's dead. If the pump header shows 0 RPM, pump's dead.
For a dead fan: Replace it. Cheap fix, $10-20. For a dead pump: Replace the whole AIO. Don't bother trying to fix it — you can't. Get a new one or switch to a quality air cooler like a Noctua NH-D15. They're quieter, last forever, and never fail.
Warning: Don't run your PC with a dead AIO pump for more than a minute. It can cook the CPU in seconds. Shut it down immediately if you see temps spike above 90°C at idle.
